Second Bachelor's degree
The courses listed above are required for the program for which the student will receive a second bachelor's degree upon completion of all academic requirements.
Associate degree
The courses listed above are required for the program for which the student will receive an associate degree upon completion of all academic requirements.
Preparatory coursework for graduate or professional school
The courses listed above are preparatory coursework required for enrollment in a graduate or professional program. Courses cannot apply to a graduate program and must be undergraduate level. Taking courses to raise GPA for program admittances does not qualify a student for federal loans and this form should not be signed. There is a one-year limit of federal financial aid eligibility for required pre-requisites. Professional degree programs must be post-baccalaureate.
Teaching Certification
The courses listed are required for elementary or secondary teacher certification or recertification in the state where the student plans to teach. Administrative, pupil services and personnel programs are not eligible.
Certificate program
The courses listed above are required for a C coded certificate program. Eligibility is based on the certificate code. Please list the 3 digit certificate code on this form in the program the student is pursuing. See your certificate academic advisor if you are not sure what the 3 digit certificate code is and make sure you are admitted to that program prior to submitting the Program of Study form.
